As an extension of the Texas Health Steps Program (EPSDT), CCP provides medically necessary services to Medicaid-eligible children (under age 21) statewide. Outreach Health Services provides hourly private-duty nursing care for chronically ill children through CCP. MDCP is a Texas Medicaid waiver program that provides respite care for families along with other services to enable medically fragile children to live at home. MDCP serves a limited number of Texas children under age 21. For more information about MDCP, call the Texas Department of Human Services at (877) 438-5658.
