The Outreach Health Services Pediatric Care program provides high quality care for children and their families in the home setting through a nursing process that includes nursing assessments, education and child/family support.  We use a multi-disciplinary approach in providing home services.  Communication between a child's family, pediatric case manager, home care nurse and physician is an integral part of the services we provide.  By working together, we can help each child attain his or her maximum potential.

Since 1995, Outreach has been providing quality home health care to Texas children.   Our case managers and pediatric nurses are available 24 hours a day, 7 days a week, to give the best care possible to our patients, families and referral sources.
