Community Care:  Primary Home Care / Family Care (PHC/FC)

Persons whose health problems cause them to be functionally limited in performing activities of daily living can benefit from our Primary Home Care/Family Care services. PHC provides non-technical, personal care tasks such as bathing, dressing, exercising, routine hair and skin care, toileting, transfer/ambulation, meal preparation and assisting with self-administered medications.

Services also can include home care management such as cleaning, laundry and shopping, escort to doctors appointments and other medical treatments.

An initial physician's order is required for participation in this program.
