Essential Duties:
Develops physical therapy plan of care involving complex modalities designed to promote maximum potential rehabilitation of Outreach Health Service’s patients.
Skill Required:
• Assists the physician in his evaluation of patients by performing muscle, nerve, joint, and functional ability to tests. • Evaluates the patient’s level of function and needs, formulates an individualized plan of care, and implements the program. • Evaluates the patient’s home for safety, offers suggestions to make the home environment safer and more accessible to the patient. • Orders equipment that will aid in increasing the patient’s mobility and independence and identifies the need for the equipment. • Observes, records, and reports to physician the patient’s reaction to treatment and any significant change in the patient’s condition. • Supervises Physical Therapy Assistants on an ongoing basis.
Minimum Qualifications:
• Physical Therapist licensed by the State of Texas. • At a minimum, hold a Bachelor’s Degree in Physical Therapy. • At least one year Physical Therapy experience required.
