This position requires a currently licensed RN in the State of Texas and maintain current Texas Driver's license. The Registered Nurse provides nursing services, treatments, and preventative procedures requiring specialized skills to patients in their home, under the direction of the attending physician.
May make initial evaluation visits and admit qualifying patients to service.
Develops and implements the patient's plan of care according to the physician's orders.
Coordinates services with the community agencies to provide proper patient care.
Is responsible for reassessment in a timely manner and documents all changes/communications in the plan of care and sends it to the physician for his/her signature.
Arranges and coordinates all ancillary services and assures that Case Conferences are performed on a timely basis with all services participating. Must be able to travel to perform field visits. Not all sites/locations will be handicapped accessible.
Bilingual-Spanish is a plus!
