Thriving at Home
Outreach Health is here to make thriving at home a reality. Our companionate care team provides the support and resources needed for seniors and those who are managing chronic health issues or just need a little extra care to live comfortably at home. Care is available up to 24 hours a day, 7 days a week. Outreach will work with each family to create a care plan and schedule tailored around the specific needs and preferences.
Personal Care Services
Outreach Health is here to support you or your loved one with assistance with activities of daily living. Depending on the needs of your family, personal care services may include meal preparation, grocery shopping, medication reminders, light housekeeping, hygiene assistance, and companionship.
Your care team will include caregivers, care coordinator, care manager, and nurses to oversee every aspect of your wellbeing. For over 45 years, Outreach has been giving families peace of mind, knowing their loved ones are receiving the best care possible.
Private Duty Nursing
Private Duty Nursing is an option for children and young adults with special medical needs such as: tracheostomy and ventilator care, feeding tubes, nebulizer treatments, total parenteral nutrition (TPN), and frequent seizures requiring intervention. Private Duty Nursing is provided in your home or child’s school or daycare.
Our goal is to provide superior care and support to children and their families, thereby maintaining children’s health while living at home. Working with families, we strive to help each child attain his or her maximum potential.
Private Duty Nursing services can be on a long-term or short-term basis as ordered by a physician, and covered by Medicaid and most insurances. Outreach Health provides Private Duty Pediatric Nursing services in the El Paso, Texas area.
Skilled Nursing Care
In addition to Home Care Services, we also provided Skilled Nursing care that can be covered by Medicaid. Our compassionate team of nurses are here to help manage complex medical needs in your home by providing:
- Medication box refills
- Medication and disease education
- Wound care
- Foley catheter care
- And so much more
